Use this as a ready-to-run toolbox talk before shifts that put anyone under cranes, hoists, or elevated equipment. It takes about ten minutes. Read it as written or make it your own — the goal is that everyone walks away treating overhead hardware as a live hazard, not background scenery.

“A U.S. worker is injured by a falling object about every ten minutes — roughly 48,000 injuries and around 250 deaths a year. Struck-by incidents are one of OSHA's Fatal Four causes of construction deaths. Today we're talking about the version of that hazard that lives right over our heads: the hardware on our cranes.”
Make it physical (2 minutes)
Hold up an ordinary bolt, nut, and washer assembly.
“This weighs about a pound. From a bench, it bruises your toe. From a runway beam sixty feet up, dropped-object charts put it in the fatality zone — and a hard hat is rated for bumps, not for steel arriving from structure height. Now think about how many of these are directly above where you'll stand today: trolley covers, sheave guards, festoon clamps, light fixtures, rail clips.”
Explain why they fall (2 minutes)
“Nothing up there is trying to stay put. Travel vibration, shock loads, and thousands of duty cycles work fasteners loose full-time. Inspections are snapshots — a bolt can back out between them. That's why the answer isn't just 'inspect more.' It's keeping hardware attached even when it loosens.”
The three behaviors we're asking for (3 minutes)
- Respect the drop zone. Never walk or work under a suspended load, and treat barricaded zones under overhead work as hard boundaries — even for a shortcut.
- Report loose or missing hardware as a near miss. A missing guard bolt didn't disappear — it fell. Say something, tag it, log it. Trend data tells us which components need engineered retention instead of endless re-torquing.
- Tether everything that goes up. Tools carried at height get tool lanyards. Hardware that lives at height gets secondary retention — tethered or captive fasteners — so a backed-out bolt hangs instead of falling.
Close with questions (2 minutes)
Ask the crew: “Where on our equipment have we found loose hardware before? What's directly above your workstation right now?” Write the answers down — that list is the start of a drop survey and your retrofit priority list.
